We joined “Jockey Club Community eHealth Care Project” in 2017. The eHealth project is the first territory-wide elderly care support project which integrates health management technology, community care and professional support.
Objectives
To apply eHealth solutions to empower individuals in health management
To promote elderly centre as the first point of contact for detecting and addressing the health and social needs of the elderly
To pilot eHealth technology to improve quality of life for the elderly and analyse the health characteristics and patterns of the elderly through big data analytics.
Service Components 1. Tele-care Programme - to empower self-management among the elderly
eHealth station at elderly centre
Health data is transferred to nurse team by cloud technology for proactive monitoring and follow-up
eHealth members can log in to the eHealth station and conduct regular measurement at the elderly centre with smart card
2. Well-being Survey - To detect the health and social needs of the elderly
Survey report is shared with the elderly centre to detect members' service needs for providing suitable services
Members can better understand their own health condition

Cognitive status
Social needs
The use of health services and medication
3. Evaluation and Big-data Analytics The Project will also apply big data analytics to examine possible health risks of the elderly and find out the association between daily health readings and the long term health-status.
